REGISTRATION REQUIREMENTS

For New, Transfer, and Re-entry Students

• 
  All students must be accompanied by a parent or legal guardian at the time of registration.

• To register, you must provide the following documentation:

  • Proof of prior school enrollment or withdrawal form from previous school
  • Copy of the student's academic transcript
  • Copy of all discipline records OR verification that there is no discipline record
  • A state requirement per Georgia State Law - Code 20-2-670
  • Proof of Residency - Two separate acceptable forms of residency
  • Custody/Guardianship documentation
  • Georgia Immunization Form #3231
  • Birth Certificate
  • Social Security Card
  • Current IEP MUST BE PROVIDED for Special Education students

If you do not have ALL of the above information, please acquire any missing information and return when you have ALL documentation.

Registration is ONLY on Monday, Tuesday, and Thursday from 8:30 AM to 3:00 PM or by appointment.
