New Student Registration Requirements

  • Certified Birth Certificate
  • Up to Date Immunization Record
  • Ear/Eye/Dental Record (EED) / Form 3300
  • 2 Proofs of Residence (1 must be a lease or mortgage agreement/1 must be a utility bill)
  • Copy of parent's photo ID
  • Copy of last year's report card showing promotion or placement in next grade level (if students are beginning Kindergarten, this is not applicable)
  • Any pertinent files for Special Education students

From the Principal

Dear Live Oak Parent/Guardian,

Words cannot express how excited we are to work alongside you this year as we embark upon our Live Oak adventure! We have a beautiful state of the art facility complete with emerging technology such as ActivBoards in each classroom as well as two fully operational computer labs.

Our goal is to provide students with a safe and nurturing environment that will enable them to academically thrive and successfully compete with others on a global level. Research shows that 21st Century Learners have the ability to work collaboratively with others, practice civility with others, and create ideas for the betterment of others and themselves. We strive to equip students with these skills in order for their success to be maximized at all times.

As parents/guardians, you are an essential part of your child’s success. It takes the community, parents, educators, and students working together to build a productive future for our children.

Please know that we are in the business of working together with families for the benefit of our children. We are proud of Live Oak and hope you will be also!  Remember, it's always a GREAT day to be a Live Oak Lion!
